WVSHP's awards will be presented at the WVSHP Annual Meeting held in the Spring.

PHARMACIST AWARDS

 

Excellence in Hospital Pharmacy

 The individual will meet the following criteria:

q        An active member of WVSHP

q       Practices pharmacy in an organized healthcare system

q       Exemplifies high standards of work performance (i.e. progressing pharmacy towards meeting ASHP 2015)

q       Has demonstrated service to their practice setting, such as implementing programs which have a positive impact on the institution and department

q       Enhances the recognition of pharmacists as medication management experts

q       Makes patients their highest priority and strives to meet their expectations

q       Exceeds goals that have been set for this individual

q       Strives to enhance the profession of pharmacy 
 

Merck Community Service in Healthcare Award

The individual will meet the following criteria:

q       An active member of WVSHP

q       The pharmacist has instituted new programs or projects that enhance the healthcare and wellness of patients. (Examples might include “brown bag programs, disease screenings, installing patient information centers, counseling booths, immunization programs)

q       Pharmacist has provided significant contributions to his or her community through non-pharmacy related activities (examples: Church, community, organizations, schools, etc.).

q       The pharmacist has an active role in volunteer services to the community.

q       Is a role model for other pharmacists and community members
 

Merck WV Young Pharmacist of the Year Award

The individual will meet the following criteria:

q       A pharmacist who has been licensed for 5 years or less

q       Exemplifies high standards of work performance

q       Provides service to the profession such as organizational involvement

q       Exceed expectations / goals set by leadership

q       Shows initiative in enhancing pharmacy as a profession
 

WVSHP Leadership Award

 The individual will meet the following criteria:

q       An active member of WVSHP

q       Committed to the delivery of quality service and the maintenance of the highest professional standards

q       Participates in public service in governmental and regulatory affairs

q       Contributes to the advancement of WVSHP and its membership both within the state and/ or on a national level
 

WVSHP Distinguished Educator Award

 The individual will meet the following criteria:

q       An active member of WVSHP

q       Individual demonstrates dedication to the health care professions by commitment to education

q       This individual may participates in precepting students, residents and other professionals or enhances the profession through teaching

q       Participates in providing additional education through professional posters, publications, presentations to professional organizations, classroom or workshop instruction
 

Pyxis Technology Award

The individual will meet the following criteria:

q       A WVSHP Active member

 Pharmacy Technician Award

 The individual will meet the following criteria:

q       A WVSHP active member

q       Demonstration of contributions and dedication to the advancement of pharmacy technician practice

q       Demonstrates contributions to the West Virginia Society of Health System Pharmacist

Self improvement efforts through attending educational programs, seminars, etc
